Bordeaux nursing home fined, suspended from admitting new patients

State's action against Signature HealthCARE's Nashville Community Care & Rehabilitation Center is related to alleged violations of administration and resident rights standards.

Nashville Community Care & Rehabilitation at Bordeaux was formerly known as the Bordeaux Long-Term Care facility.

The state has suspended the former Bordeaux Long Term Care facility from admitting new patients and appointed a special monitor to review that nursing home's operations. 

Louisville, Ky.-based operator Signature HealthCARE LLC also has to pay two state civil monetary penalties totaling $7,500, according to the Tennessee Department of Health.

The orders are related to conditions found at the 419-bed facility now called Nashville Community Care & Rehabilitation Center during a complaint survey conducted from Sept. 25 to 28.

Specifically, surveyors found violations of administration and resident rights standards during the inspection. The investigation was completed on Oct. 19.

Signature HealthCARE runs Nashville Community Care & Rehabilitation Center at 1414 County Hospital Road under a four-year lease with Metro that runs through 2020. Under that agreement, Metro reimburses the post-acute care provider up to $3.5 million a year for financial losses with the city responsible for any needed life safety improvements.

Nashville Community Care & Rehabilitation Center has the right to a hearing regarding the suspension of admissions before the Board for Licensing Health Care Facilities or an administrative judge, according to a press release from the state's health department.

Signature HealthCARE' lease on the former Bordeaux Long-Term Care facility was extended for another four years through 2020.

State health commissioner Dr. John Dreyzehner can suspend admissions to a nursing home when conditions are deemed to be, or are likely to be, detrimental to the health, safety or welfare of the residents, the release read. "The order to suspend admissions remains effective until conditions have been and continue to remain corrected," it read. 

The last time admissions were suspended at Nashville Community Care & Rehabilitation Center, which was then known as the Bordeaux Long-Term Care facility, was in 2007.
