In his latest column, KFF President and CEO Drew Altman examines the implications of Secretary Kennedy’s reorganization of HHS and why it’s a sharp break from past efforts to reorganize the department, noting: “Prior reorganizations at HHS were mostly dedicated to building new capabilities and the federal role in health…Secretary Kennedy’s reorganization…represents an effort to cut back the size and role of the department as part of broader administration-wide efforts to reduce the role of the federal government in health.”

Enrollment Growth in the Affordable Care Act Marketplaces

 

Our analysis of enrollment in Affordable Care Act Marketplace health plans finds a record 24.3 million people enrolled in 2025--more than double the total in 2020--with most of the growth occurring in states won by President Trump in the 2024 election. In six states, enrollment more than tripled since 2020.
